Measure with the child standing, weight on the foot, heel against a wall, and take the distance to the longest toe. Do both feet and use the longer. Because Mondopoint is a direct measurement, a boot two sizes up is genuinely two centimetres of unoccupied space, which is a great deal more slop than the same nominal jump in a shoe.

Fit is judged flexed forward, not standing upright
The commonest fitting error is assessing a ski boot the way you would a shoe — child stands up straight, parent presses the toe. In that position the foot slides forward and the toes touch the front, and everyone concludes the boot fits.
Skiing does not happen in that position. Buckle the boots, have the child flex their knees and shins forward against the tongue as if starting a run, and then check. Flexed forward, the heel should seat back into the heel pocket and the toes should pull back to just brush the front. If the toes are still crushed when flexed, the boot is too short.
